Name the season, episode, method, and who was the perpetrator each time Paige was either killed or nearly killed. If she survived, list the method of salvation.

Season 6 Soul Survivor
Unlike many of the other Halliwell deaths, this was planned by the "victim". Paige barters her soul in order to free Larry's soul, obtained in a Faustian pact by Zahn, a lower level demon who has quietly been trading demonic powers for the bartered souls. Paige's plan is to gain access to key information, namely, where the Faustian contracts are kep. Paige hopes to free all of the lost from their contracts with Zahn so that they may move on. For Zahn's part, the prospect of gaining a Charmed soul in return for a mere mortal's - Larry's - is a deal perhaps too good to be true. He salivates at the prospect of acquiring many more demonic powers by trading Paige's soul.
Before it's too late, Richard has to clue Piper and Phoebe into Paige's plight. They arrive in the nick of time as Paige is fading away. Piper's blasting of the storage vault at Paige's dying behest sets the contracts ablaze and returns Paige to the land of the living.
Season 6 Sword and the City
Paige makes it two episodes in a row as she succombs to death yet again. This time neither she nor Phoebe plan to be electrocuted by an anonymous demon , who, fearing a trap, attacks the Charmed Two as they try to dissuade Piper from embracing Excalibur and becoming a witting agent of a demon.

Season 6 should have a subtitle: Death Becomes Them. In the event, Paige, according with Phoebe, almost die or at least becomes ghosts in I Dream of Phoebe as the master (an upper level demoness commands Richard to kill the Charmed Ones. Only because Leo heals an equally dying Piper do Paige and Phoebe remain ghosts until they can get the demoness back into the bottle. Their plan succeeds as Phoebe possesses the demoness and commands that the Charmed Ones be brought back to life. Death becomes them indeed!

Paige's death and near death experiences by season: Season 4 Season 5
Centennial Charmed The real Paige, of course, does not die. However, in the alternate universe that Cole has created, Paige is dead. Presumably, Cole was responsible for her death, as he knows where she is buried. And, the Source does not seem to have bounty hunters after Cole any longer, although he does have the Seer keeping an eye on Cole. Of course, Phoebe failed to read the warning in Arabic that the genie trapped inside was really a powerful demon. Once free, Gini wishes the Charmed Ones dead. Paige and Phoebe become ghosts, but cannot move on because Leo manages to heal Piper despite the deep sleep into which Chris has wished both Piper and Leo into. Vanquishing Gini allows Richard to wish the Charmed Ones back to life. Paige - and Phoebe - return to the land of the living. Season 7 Styx Feet Under The ever-changing list of people to die finally nabs Paige. Her name is one that neither Phoebe nor she suspects is on the list. Yet, it is. And, worse: Paige actually dies. Piper, forced by the Angel of Death to be his assistant, guides Paige to the white light that will guide her to the afterlife. All this comes to naught as fate has other things in store for Paige. Season 8 Kill Billie, Volume 2 Paige is missing and presumed dead after the events of the Ultimate Battle Mark 1 have taken place. Since Piper manages to undo this version of the Ultimate Battle, the question of what happened to Paige becomes moot.

Paige's death in season seven's Styx Feet Under was the earliest point in any season in which a sister had died...fifth episode.
The only "deaths" earlier were Phoebe's future death and the clones of Prue (and in all of those, the deaths were not legitimate).

Paige is missing and presumed dead after the events of the Ultimate Battle Mark 1 have taken place. Since Piper manages to undo this version of the Ultimate Battle, the question of what happened to Paige becomes moot. I've always hated that Piper and Leo never looked for Paige. She could've orbed from the blast, or, just as Piper survived, Paige could've survived and been under some wreckage somewhere, too. As far as I'm concerned, that episode was just another chance for Kern to again cut Paige's role in the show.
